IF YOU ARE A PROSTATE CANCER SURVIVOR
or currently in treatment for the disease, one of the world's leading research centers urgently needs your participation in an important new survey. Clark Atlanta Cancer Center is conducting a research project on prostate cancer. They are asking for prostate cancer survivors to support this research project by completing a short (15 minute) confidential online survey. It doesn't matter what procedure you had, or how long ago you had your procedure.
How to get started:
- Click on “Create an account for Survey”.
- Fill out this page and put in your email address for the “User Name” and make up a password that includes minimum 8 letters, 1 capital letter and 1 number.
- Click “I Accept” in the required box at the bottom of the page.
- This should bring you to the survey.
